If there is not an overwhelming amount of skin and fatty tissue, then commonly the incision can extend from the bony prominence of the hip, also known as the anterior superior iliac spine, on the right side to the one on the left. If there is a large amount of excess skin and excess fatty tissue, then sometimes that scar needs to be extended beyond it and towards the sides or even the back. We try to keep the scar as low as possible. We place it generally just above the level of the pubic hair line in order to allow patients to ideally wear a bikini or a two-piece swimsuit after surgery, and to be able to conceal their scar with either a bikini or underwear. So the length of the scar will vary from patient to patient based on the amount of skin and the amount of fatty tissue that is present in the lower abdomen.
