Many photographers commonly think that any image that isn't sharp is due to missed focus. While this certainly can occur, numerous other problems may be more likely. One of the biggest mistakes I see photographers make that results in blurry images is picking the wrong shutter speed. Shutter speed adjusts how long the camera is recording data on your image, and if left open too long, it will result in blurry photos. For scenes with a moving subject or background, you'll need to pick a faster shutter speed to freeze the motion. How fast the shutter speed needs to be will depend on how quickly the subject moves, but always err on the side of too fast, rather than too slow. If neither shutter speed nor focus is your problem, it may be poor-quality equipment. That said, this is almost certainly NOT your problem if you're shooting with a modern camera and lens. Finally, you may need to clean your camera's sensor or lens. Grab a lens cloth and some lens cleaning solution and go to work.
