This was a part of our End of Life Education Series that took place in June of 2022. Advance Care Planning: Advance Directives, POLSTs and After-death Care Directives W/ Angela Franklin QMHA EOLD Learning Objectives: 1. describe key elements of advance care planning 2. understand the difference between an advance directive, a living will, and a POLST. 3. learn strategies to engage in ACP conversations 4. explore after-death care directives and identify different types of final disposition. Angela Franklin, QMHA, EOLD, is an Older Adult Specialist and Suicide Prevention Coordinator with Options for Southern Oregon, serving Josephine County. She is a Respecting Choices® First Steps/Advance Steps ACP Facilitator and Instructor. Angela is a practicing Death Midwife and Home Funeral Guide. She has worked with older adults as a caregiver, hospice volunteer and community educator. She is a founder of Journey Home DeathCare, a community-driven organization based on practical death care, education, outreach, and community grieving support.
